This course was developed in consultation with the Australian College of Midwives & remote and rural health practitioners, following an identified need for a short course that specifically targeted maternity emergency care for non-midwives working in a remote or isolated setting.
Factors of geographical isolation, minimal staffing and poor transport results in delays in transporting patients to definitive specialist care, which necessitates core skills at the remote or isolated location to care for the pregnant patient - antenatal, intrapartum and post partum, in some cases for extended periods of time.
Declining obstetric services in rural and remote areas result in a generalist work force having to respond to specialist needs. The situation is compounded by the fact that the expectations of many remote consumers have changed. Some pregnant women are choosing to birth outside of regional tertiary centres.
This course would improve the quality of health services for the pregnant woman in remote and isolated areas, thereby improving overall health outcomes.
Graduates of this course will be able to:
* demonstrate the principles of managing first line emergency care to a woman in labour and
immediately following child birth
* demonstrate the principles of managing a unexpected normal birth in an isolated setting
* identify appropriate responses to the following maternal and neonatal complications
- vaginal bleeding in pregnancy
- birth asphyxia
- post partum haemorrhage
- hypertension
- pre-eclampsia
- maternal trauma
Target group:
Health professionals including Registered Nurses, Enrolled Nurses, Aboriginal Health Workers, Torres Strait Islander Health Workers, Paramedics and isolated practice General Practitioners practicing in isolated circumstances without the resources and assistance of formal or regular obstetric and midwifery services.
Area of implementation:
Across all states and territories in remote locations or regional centres as defined by the target group, allowing for maximum access.
Activity:
Course delivered in the form of a workshop, a combination of lectures and skill stations held over 2 & 1/2 days from Friday at 3pm to Sunday 5pm. Delivered by a core group of facilitators comprising specialist midwives, midwives practicing in a rural/remote communities and/or general practice obstetricians.
Lectures include:
- normal birth
- antenatal assessment
- cultural safety
- bleeding in pregnancy
- diabetes in pregnancy
- the unwell pregnant woman
- PPH (post partum haemorrhage)
- hypertension in pregnancy
- routine care of mother and baby
- uncommon events (breech birth, cord prolapse, shoulder dystocia)
- neonatal resuscitation
